Five Wishes Five Wishes is an easy-to-complete living will. By checking a box, circling a direction or writing a few sentences, you can record your wishes for:
1) The person or persons you want to appoint to make health care decisions for you when you can’t speak for yourself. 2) The kind of medical treatment you want or don’t want. 3) How comfortable you want to be. 4) How you want people to treat you. 5) What you want your loved ones to know.
Five Wishes is legally recognized in 40 states and the District of Columbia and has already been completed by over eight million Americans. It can be completed by anyone 18 years or older who is of sound mind and acting freely.Five Wishes States
